Cheerleading - Forms
Every applicant must submit by Monday, March 18, 2013, the 2013-14 application, the medical history questionnaire, the medical physical examination and the proof of medical insurance (Student-Athlete Insurance" form) with a photocopy of the health insurance and prescription drug card(s) which cover(s) you. NO FORMS WILL BE ACCEPTED AFTER MONDAY, MARCH 18, 2013, FOR THE 2013-14 TRY-OUT. Exceptions for late forms must be approved by program Head Coach, Julie Horine. 2013-14 Application in PDF Format 2013-14 Medical Form in PDF Format Medical Forms will not be available at the February clinic. Please print all forms from the above links. If you have trouble printing any of the forms, please contact IU Sports Medicine at (812)-855-4509. All medical forms are due no later than March 18, 2013 2. Applicants must have the following information turned into IU Sports Medicine to be considered for our teams: Tryout application, the medical history questionnaire, the medical physical examination and the proof of medical insurance (Student-Athlete Insurance" form) with a photocopy of the health insurance and prescription drug card(s) which cover(s) you. Original copies of all forms are required. Fax copies are fine to meet deadline date, but originals must follow via a trackable mailing method. To fax information, please send that to NITA KING at (812) 855-1810. Once you have mailed or faxed in your medical information, please email Julie Horine at jhorine@varsityspirit.com to notify the program of your paperwork having been sent.
